A loop check record is the commissioning document that carries one row per control loop through calibration, end-to-end loop test, and DCS or PLC verification, with the technician and date against each sign-off and an open-items column for anything found. It is the evidence handed to the operating company at mechanical completion, and the record an auditor reads when asking whether a loop was proven before startup.
